24/7 nursing on call

A nurse is available around the clock every night, every weekend, every holiday. When something comes up at 2am, you won't be waiting for a callback or navigating a phone tree. You'll reach someone who knows your loved one.

Help with daily living

We assist with the activities of daily living, the everyday tasks that become harder with age, so residents can live comfortably, safely, and with their dignity fully intact.

Bathing

Gentle, respectful assistance with bathing and personal hygiene, tailored to each resident's preferences and comfort level.

Dressing & Grooming

Medication Management

Transportation & Errands

When residents need to get out for an appointment or errand, we handle it. Most care, including our nurse practitioner, physical therapy, and pharmacy, comes directly to the house.

We know who needs encouragement to drink, who prefers warm tea, who will finish a glass if someone sits with them. Dehydration and weight loss are preventable. We prevent them.

Medication management you can trust

Missed or incorrect medications are one of the leading causes of health complications in seniors. At Lavender Fields, our licensed staff and AMAP-certified caregivers administer and track every prescription, coordinate with Davis Specialty Pharmacy in Elkins, and flag any concerns to the on-call nurse immediately. Good coordination means fewer gaps, fewer errors, and fewer medications that pile up over time without anyone asking why. We are not anti-medication. We are anti-chaos.

Getting there without the stress

Most care comes to us. Melissa Phares, our nurse practitioner from Davis Medical Center, sees residents here at the house. Physical therapy comes to us. No arranging rides, no navigating unfamiliar buildings, no steps to climb. Davis Specialty Pharmacy in Elkins delivers directly. When a resident does need to go out for an appointment or a personal errand, we handle the transportation reliably, safely, and with a familiar face behind the wheel. Families do not have to rearrange their schedules.

Meals together. Weight that comes back.

We sit together at the table for every meal. Not alone in a room. Not on a tray in a hallway. That matters more than most families realize. Shared mealtimes mean residents eat more, drink more, and stay connected to the people around them. For elders who have been struggling to keep weight on, this is often one of the first things families notice changing.

Hydration is something we actively watch. Dehydration in seniors is easy to miss and quick to cause serious problems. Our staff knows who needs encouragement to drink, who prefers warm tea over water, and who will finish a glass if someone sits with them.
