Lavender Fields is a 16-bed home in Beverly, West Virginia. We have cared for seniors here since September 2015. Ten years on, our record speaks for itself: zero pressure ulcers, zero chemical restraints.


We are a family, not a facility. Our residents live in their own bedrooms, share meals together, and move through their days with people who know their names, their stories, and their needs.



We provide specialized care for dementia, Alzheimer's, and chronic illness. Our approach is personal, never rushed. Every resident is seen as a whole person, not a diagnosis.

Lavender Fields was built on a simple lesson: people remember how they were treated, even when they forget everything else.


Years ago, I found Dora, an Alzheimer's patient, wandering in the cold. She did not know where she was. She did not know her own name. But she remembered kindness.

That moment shaped this entire facility.


We do not rush. We do not reduce people to their diagnosis. We sit with our residents. We learn their stories. We hold them, all the way through.

A Personal Journey: My Story

Welcome to Lavender Fields. My name is Jewels Rose, and my journey toward creating this haven of compassionate care began with my own battle against chronic neurological Lyme disease. For decades, this illness affected every part of my life, mimicking conditions like Parkinson’s and MS. I often found myself treated as a diagnosis rather than a person, and I know firsthand the toll that can take on someone’s well-being. But everything changed when I was referred to Dr. Joseph Jemsek, a Lyme disease specialist whose approach to care saved my life—twice. Dr. Jemsek’s philosophy wasn’t just about treating the illness; it was about building connections, spending time, and truly seeing the individual behind the diagnosis. His care made me realize that healing is about more than medicine—it’s about understanding the person as a whole. This experience laid the foundation for Lavender Fields, where our care is personalized, never rushed, and always focused on treating each resident with the dignity they deserve.


My passion for dementia and Alzheimer’s care comes from my own experiences with loved ones who received impersonal, efficiency-driven care. One pivotal moment in my caregiving journey was when I found Dora, an Alzheimer’s patient, lost and wandering in the cold. She couldn’t remember where she was, but what struck me most was that, despite her memory loss, she remembered kindness. That moment changed me forever—it reaffirmed my belief that, even when memory fades, compassion leaves a lasting imprint. Lavender Fields was built on this belief. Here, we provide compassionate, individualized care that always puts our residents first. Whether they are dealing with physical ailments or memory loss, we treat each person with respect, patience, and love, creating an environment where they feel valued and supported.


Lavender Fields wouldn't be possible without the dedicated team around me. Our lead caregiver, Rebecca Marks, with 23 years of dementia care experience, helped shape our Jewel CARE Model, a holistic approach to resident care. Alongside our team, including co-owner Brett McClain, we have created a home-like environment where every resident is treated with dignity and respect. Together, we’re committed to providing a model of care that prioritizes human connection, independence, and joy for our residents and their families.

Philosophy of Care at Lavender Fields

Our care model is built on three things: presence, patience, and touch. Every resident gets attention that is personal and unhurried. Every staff member is trained in dementia care. Every interaction is guided by the same standard.


Health monitoring is steady and built into daily life:


  • Vital signs tracked on schedule
  • Weight checks at regular intervals
  • Monthly nursing assessments
  • Care plans adjusted as needs change


Dementia Care

We support residents through every stage of dementia, from mild cognitive decline to advanced disease. Our program focuses on engagement, not management. Activities are matched to each resident's abilities, and the environment is built to keep them safe without taking away their freedom.


The goal is simple. Help residents hold on to what they have. Honor what they have lost. Make the day worth living.


End-of-Life Care

When a resident enters their final season, we walk with them. We walk with their families too.



We make space for family to gather. We make room for what matters: presence, prayer, music, memory, silence. We follow the resident's lead and the family's wishes.


Ten years in, no resident at Lavender Fields has died alone.

The motivation of this facility is truly compassion. My mother lived there for 3 months. As you sit in the living room and watch the staff interact with the residents the compassion is quite obvious.   Everyone from management to the staff truly love the folks that live there.  I wholeheartedly recommend this facility. They could very easily change the name to Loving Fields.

My mother was a patient at Lavender Fields and it is the “best” place my mother could have been during this time. Judi, Brett and the staff are so caring and loving. She could walk around when she couldn't sleep, eat ice cream when she wanted, and they even celebrate with punch. The school children come from the Beverly School to read to the residents. Last but not least, the residents truly care for each other like extended family.

My uncle will be 95 this week and he's a resident at Lavender Fields. My granddaughter and I celebrated with him and others there last week. The staff as always was so kind and accommodating as we decorated and celebrated his special day. . Thanks for all you do for our Uncle Bud
